De trein naar Samarkand speelt zich af in 1923 tegen de achtergrond van de immense, 5 miljoen levens eisende hongersnood na de Russische Burgeroorlog. Dejev, de zachtaardige treincommandant, en Belaja, de harde en wilskrachtige regeringsvertegenwoordigster, hebben de opdracht 500 ondervoede weeskinderen te evacueren. Ondanks hun botsend temperament doen ze alles om hen te beschermen tijdens de zes weken durende treinreis door het hongerende Wolgagebied, de Kazachse steppen, de woestijnen en de bergen van Turkestan.
